What This Builds

A Custom GPT configured specifically for reviewing AI-generated transcription drafts in your specialty, trained on the most common error patterns Dragon Medical One and M*Modal make in cardiology, radiology, orthopedics, or whichever specialty you focus on. Instead of reviewing AI drafts with only your memory and habits as your guide, you have a systematic AI assistant flagging the exact categories of errors you're most likely to miss under production pressure.

Prerequisites

  • ChatGPT Plus subscription ($20/month, Plus), required for Custom GPTs
  • At least 1 month of experience reviewing AI-generated transcription drafts in your specialty
  • A list of error types you've personally encountered (even rough notes work)

The Concept

A Custom GPT is like a specialized coworker who only knows one job, reviewing AI transcription drafts, and knows it deeply. You train it once with your specialty's common AI error patterns, and then every time you open a new conversation in that GPT, it's already focused and ready. Unlike a generic ChatGPT conversation where you have to re-explain the context every time, the Custom GPT starts from your established setup automatically.


Build It Step by Step

Part 1: Access the Custom GPT Builder

  1. Go to chatgpt.com and sign in to your Plus account
  2. Click on your profile picture → My GPTs
  3. Click Create a GPT (green button)
  4. You'll see two panels: a chat-based "Configure" interface on the left and a preview on the right

What you should see: A builder interface with a friendly "GPT Builder" assistant asking what you want to create.

Part 2: Configure Your Draft Review GPT

Instead of using the chat-based configurator, click the Configure tab at the top for direct control. Fill in each field:

You are a medical transcription quality review assistant specializing in [specialty] documentation.

Your job: When I paste text from an AI-generated transcription draft, review it and flag any likely errors. Do NOT flag correct text. Only flag items that are genuinely suspicious.

Flag these categories:
1. HOMOPHONES: terms that sound right but may be wrong (ileum vs ilium, discrete vs discreet, peri- words)
2. DRUG ERRORS: dosages that seem implausible for the route/condition, drug names transcribed phonetically
3. LATERALITY: missing or incorrect left/right/bilateral designations
4. ABBREVIATIONS: expanded incorrectly or used in wrong context for [specialty]
5. NUMBERS: measurements, lab values, or dosages that fall outside typical ranges for this specialty
6. ANATOMY: anatomical terms that may have been swapped for similar-sounding terms
7. IMPLAUSIBLE PHRASES: complete sentences where the clinical meaning doesn't make sense

Output format: For each flagged item, give:
- The flagged text (quoted)
- The category from the list above
- Why it's suspicious
- The most likely correct version

If nothing is suspicious, say "No flags, draft looks clean."

CRITICAL RULES:
- Never assume the text contains PHI. I will only paste sanitized, de-identified text
- Do not rewrite the entire document. Only flag suspicious items
- Be conservative: only flag items you're genuinely uncertain about, not every term

Conversation starters (add these):

  • "Review this cardiology draft for errors"
  • "Flag any suspicious terminology in this text"
  • "Check this operative note draft"
  1. Click Save and set visibility to Only me

What you should see: Your Custom GPT appears in "My GPTs" with your specialty name.

Part 3: Test and Refine

  1. Open your new Custom GPT from "My GPTs"
  2. Generate a test draft using the Level 1 anatomy primer prompt, then deliberately introduce 3-4 common errors (wrong homophone, wrong laterality, wrong dosage)
  3. Paste the test draft and ask it to review

What you should see: The GPT flags your planted errors with clear explanations. If it misses planted errors or flags correct text, go back to Configure and refine the Instructions.

Calibration questions to run:

  • "Why did you flag [term]? It looks correct to me." (refines false positive rate)
  • "You missed [error type]. Add this to your review checklist." (updates your instructions)

Real Example: Cardiology Operative Note Review

Setup: You've configured a Cardiology Transcription Draft Reviewer with cardiology-specific error patterns.

Input (sanitized, no PHI):

Output from your Custom GPT:

  • FLAG (Homophones/Spelling): "florescent," likely "fluoroscopic" in this surgical context
  • FLAG (Spelling): "Transeptal": standard spelling is "transseptal"
  • FLAG (Spelling): "devise", should be "device" (verb vs. noun confusion)
  • FLAG (Anatomy): "right atrial appendage": the WATCHMAN device closes the left atrial appendage (LAA), not the right. This is a significant clinical error.

Time saved: Catching the "right/left atrial appendage" error manually requires deep cardiology knowledge under time pressure. The GPT flags it systematically in 10 seconds.


What to Do When It Breaks

  • Flags too many false positives → Add to Instructions: "Reduce sensitivity, and only flag items where you are 70%+ confident there's an error"
  • Misses errors you're finding → After your shift, go to Configure and add: "Also flag: [the error type you found]"
  • Responses get generic → Start fresh conversations instead of continuing long threads; Custom GPTs work best in fresh sessions
  • Can't handle your specialty's terms → Upload a PDF of common specialty terminology to the Custom GPT's knowledge base (Configure → Knowledge → Upload files)

Variations

  • Simpler version: Instead of a Custom GPT, save a detailed review prompt as a ChatGPT "custom instruction" and paste it at the start of each review session
  • Extended version: Upload your facility's style guide PDF to the GPT's knowledge so it can also flag formatting errors, not just terminology

What to Do Next

  • This week: Build the GPT, test it with 5 real (sanitized) draft samples, and calibrate the sensitivity
  • This month: Use it for every AI draft you review; keep a log of errors it catches vs. misses to continue refining
  • Advanced: Build a separate GPT for each specialty you work in, since a radiology reviewer has very different error patterns than a cardiology reviewer
